On November 12, 2013, the NRC issued a Notice of Violation to Southwest X-Ray Corporation for a Severity Level III Problem involving failure to implement 10 CFR 34.41(a), requiring a second qualified radiographer to be present during radiographic operations at locations that are not permanent radiographic installations, and 10 CFR 34.46(c), requiring a radiographer's assistant using radiographic exposure devices to be under the personal supervision of a radiographer. Specifically, on July 8, 2013 the licensee failed to ensure that its radiographer's assistant was under such supervision when the qualified radiographer remained in the office during radiographic operations in Casper, Wyoming.
